1. Disclosure of Behavioral History

By signing below, I, the undersigned client, agree to fully and honestly disclose any and all known behavioral issues my dog may have. This includes, but is not limited to, aggression toward humans or other animals, resource guarding, fear-based behaviors, reactivity, or bite history. I understand that accurate disclosure of such behavior is essential for the safety of all people and animals involved in training.

2. Liability for Injuries or Damages

I understand and agree that I am fully responsible for any injury or damage caused by my dog while on the premises of Willow Creek Dog Training or while participating in any activity organized by Willow Creek Dog Training, whether on-site or off-site.

Should my dog bite, injure, or cause harm to any Willow Creek Dog Training employee, volunteer, guest, or another dog during training activities, I agree to be held financially responsible. *

3. Responsibility for Guests

A. I understand that I am responsible for the actions, behavior, and safety of any person I bring onto Willow Creek Dog Training property, including but not limited to family members, friends, or other guests.

B. I agree that anyone under the age of 12 is not permitted on the property without prior written or verbal approval from Willow Creek Dog Training. If approval is granted, the child must remain under direct supervision of the adult responsible at all times.

4. In-Home Training Liability

If I schedule in-home lessons with Willow Creek Dog Training, I understand and agree that I am responsible for maintaining a safe and secure environment for the trainer during each visit. This includes properly securing all animals not participating in training, disclosing any potential hazards in the home or on the property, and ensuring the dog being trained is under control at the time of the trainer’s arrival.

5. Photography and Social Media Release

I understand that photographs and/or videos of my dog may be taken during training sessions or while on the premises. These images may be used by Willow Creek Dog Training for marketing, promotional, or educational purposes, including but not limited to social media, websites, printed materials, and online advertising.

6. Assumption of Risk

I acknowledge that working with dogs, particularly those with behavioral challenges, carries inherent risks. I agree to assume full responsibility for any and all risks, injuries, or damages that may occur as a result of participating in training sessions with Willow Creek Dog Training.

7. Hold Harmless Agreement

I agree to hold harmless and indemnify Willow Creek Dog Training, its owners, employees, contractors, and volunteers from any and all claims, damages, costs, and liabilities arising from or related to my dog’s participation in training sessions, presence on the property, in-home visits, or actions of any guests I bring.

I agree my dog is healthy, up to date on all core vaccinations for their age, and free from and fleas, ticks or parasites.

Cancellation & No-Show Policy

The client understands that if they do not cancel their scheduled lesson at least 4 hours in advance, they will be responsible for a $25 late cancellation fee.

If the client fails to call or does not show up for the scheduled lesson without notice, they will be charged a $50 no-show fee.
